How we treat sciatica to restore function
As a chiropractor, I regularly see patients seeking help for sciatica – a pain that radiates from the lower back down into the buttocks, legs and sometimes even down to the foot. Sciatica is not a diagnosis in itself, but a symptom of the sciatic nerve being irritated or compressed, often due to a herniated disc, muscle tension or joint locking. It can be a severely limiting pain, but there is hope – and at our clinic we have the solution.
Chiropractic and napropathic adjustments are often the basis of treatment. If the spine is misaligned or the joints are locked, this can increase pressure on the sciatic nerve. Through specific adjustments, we restore mobility and balance in the lower back and pelvis, which relieves the nerve and reduces inflammation. This is a safe and effective method that targets the cause – not just the symptoms.
In combination with adjustments, we use soft tissue therapy such as massage to treat tense muscles, especially in the buttocks region, where the piriformis muscle often plays a role. Piriformis syndrome is a common cause of sciatica-like pain, and through deep muscle treatment we reduce pressure on the nerve and improve circulation. This often makes a big difference to the patient’s pain and ability to move.
For some patients, we supplement with dry needling – an evidence-based technique where we use thin needles to release deep trigger points in the buttocks and lower back. This releases muscular tension that can otherwise compress nerve tissue and increase pain. Many experience rapid relief after the first treatment.
We also work with neuromotor activation and functional rehabilitation. Once the pain has decreased, it is important to reactivate the muscles that have been inhibited by nerve damage. Exercises that focus on core stability, hip function and posture help prevent relapse and strengthen the nervous system’s feedback. We guide each patient through an individually tailored exercise program.
Finally, we use lighter traction/mobilization techniques, especially in cases where a bulging disc or herniated disc is the cause. By gently separating the vertebral segments, we reduce pressure on the discs and nerves, which provides space for healing and pain relief. This is especially effective in acute cases where traditional mobility training is not yet possible.
